Aztec High School

Aztec High School is looking forward to hosting a Covid Vaccine Clinic on Thurs, Sept 16th in the Lillywhite Auxiliary Gym 4-6pm. 
The event is now open in the vaccinenm.org portal and people can start scheduling appointments.

• We will be offering Pfizer vaccine only. (EUA for ages 12+, FDA-approved for ages 16+)
• A written/signed consent is required for minors (< 18 years old) are NOT accompanied by a parent/legal guardian.  In other words, if the parent/legal guardian brings the child to the event, we don’t need a written consent form.
• Minors who are NOT accompanied by a parent/legal guardian, will need to remain in observation for 30 minutes.
