| STUDENT REGISTRATION INFORMATION (2007-2008 School Year) |

| Entry Age Kindergarten –
All children entering Kindergarten in public schools must be five years
old by 12:01 a.m. on September 1, 2007.
Entry Age First Grade – All children entering first grade must be six years old by 12:01 a.m. on September 1, 2007. A copy of the child’s birth certificate and immunization record must be on file in the principal's office PRIOR to the student attending class. Please bring proof of immunizations, residency, birth certificate and social security number and/or census number to registration. | New Mexico State Law requires
the following for school entrance:
1.DPT* - Appropriate immunization against diptheria-tetanus-pertussis. After the 7th birthday, students are exempt from pertussis. 10-year booster doses must be up to date. 2. Polio* - Three doses of Polio vaccine given at appropriate intervals. 3. MMR* - Two doses of Measles vaccine, one dose of rubella and one dose of mumps. Doses received prior to 12 months of age do not count. 4. Hepatitis B – Completion of three doses of Hepatitis B for all students starting the 7th grade and preschool. 5. Varicella – Students entering preschool and kindergarten must have a reliable history of chickenpox or one dose of Varicella Vaccine after the first birthday. *Preschool students will follow a slightly different schedule per New Mexico requirements. It is a violation of the Compulsory School Attendance Law for a student to enroll who is not appropriately immunized, in the process, or properly exempted from immunization. Aztec Schools will not allow a grace period of conditional enrollment to prove immunization status. An original immunization record must be presented for recording by the school upon the student’s enrollment. Students exempted from the above requirement for religious reasons must, upon registration, submit a completed and notarized “Certificate of Religious/Conscientious Objection to Immunization” form. The school nurse will submit this certificate to the State Immunization Program for approval. Students exempt from the above requirement for medical reasons, upon registration, must submit a statement from a licensed medical/osteopathic doctor. The school nurse will file medical exemptions in the student’s cumulative folder after approval. Both medical and religious exemptions do not exceed the school year for which they are submitted and approved; they must be renewed annually. |

| Records of transferring
students are released to receiving schools as a matter of normal practice.
The Aztec School District will release student records to other schools
without written consent of the student or parent. If new to the district
please bring proof of immunizations, residency and child’s birth certificate
to registration. All students need to provide their social security
number and/or census number to the district.

| PRESCHOOL SCREENING | ||||||||||||
| Screening for new preschool students will be held in August. Please call Lydia Rippey Elementary or McCoy Elementary for an appointment. | ||||||||||||
| ELEMENTARY SCHEDULE | ||||||||||||
| All students entering Preschool though grade 5, whether or not they have attended Aztec Schools, must register at Park Avenue, McCoy or Lydia Rippey on one of the dates indicated. All Preschool – 3 students residing on the east side of the Animas River will attend school at McCoy Elementary. All Preschool – 3 students residing on the west side of the Animas River will attend school at Lydia Rippey Elementary. Park Avenue serves all students in grades 4 and 5. | ||||||||||||

| AZTEC HIGH SCHOOL SCHEDULE | ||||||||||||
| Students new to the district
and students who are returning from a disenrollment procedure must register
Tuesday or Wednesday. All other registered students may pick up their
schedules on August 9, 10 and 13.

Students and parents/guardians please report to the Administrative Offices. Students with past due fees, or those lacking proof of immunizations will not be permitted to register or pick up their schedules. Parents are encouraged to provide students with money to pay for current fees. The Aztec High School office will gladly provide you this information by telephoning 334-9414.
